The COVID-19 self-assessment was updated on July 16th – this update includes vaccination symptoms.
It includes updated questions that consider side effects that may happen in the 48 hours after getting a vaccine. It can be used for both visitor and staff sign-in sheets. Please note that it has been updated to include vaccination-related symptoms, specifically headaches, fatigue and muscle aches/pains that are “not related to getting a COVID-19 vaccine in the last 48 hours”.
